TRADITIONAL FUNERALS
The goods and services listed are those we can provide to our families. You may
choose only the items you desire. However, any funeral arrangements you select
will include a charge for our basic services and overhead. If legal or other
requirements mean you must buy any items you did not specifically ask for, we
will explain the reason in writing on the statement we provide describing the
funeral goods and services you selected. You will note that we offer
Personalized Service Program Packages. The funeral director handling your
arrangements will further explain our Personalized Service Program Packages.
PERSONALIZED SERVICE PROGRAM PACKAGES It is our experience that most families
select one of the following services.
I. COMPLETE TRADITIONAL SERVICE/CEREMONY
This service includes: the basic professional services of the
funeral director and staff, administrative staff and professional staff for
coordinating and directing the funeral; filing and obtaining legal documents
and other paperwork necessary to complete the arrangements; removal of the
deceased from the place of death to the funeral home within a 25 mile radius;
embalming and/or sanitary care of the remains; dressing, casketing,
cosmetizing, use of the facilities for visitation: use of the facilities for
the service at the funeral home, automotive equipment. (safety/lead vehicle,
funeral coach, flower/equipment vehicle, errand/service vehicle, family car),
memorial register, memorial video, memorial folders and acknowledgment cards.
This service does not include: merchandise selected such as
casket and outer burial container.
2. GRAVESIDE SERVICE/CEREMONY
This service includes: the basic professional services of the
funeral director, administrative staff, and professional staff for coordinating
and directing the funeral; filing and obtaining legal documents and other
paperwork necessary to complete the arrangements; removal of the deceased from
the place of death to the funeral home within a 25 mile radius; embalming
and/or sanitary care of the remains; dressing, casketing, cosmetizing. use of
the facilities for visitation; use of equipment for the service and set up at
the graveside; automotive equipment (safety/lead vehicle, funeral coach,
flower/equipment vehicle, errand/service vehicle, family car), memorial
register, memorial video, memorial folders and acknowledgment cards.
This service does not include: merchandise selected such as
casket and outer burial container.
3. IMMEDIATE BURIAL
This service includes: the basic professional services of the
funeral director, administrative and professional staff for obtaining legal
documents; removal of the deceased from the place of death to the funeral home,
within a 25 mile radius; refrigeration; sanitary care of the remains;
transportation of the deceased to the cemetery.
This service does not include: embalming, viewing, or use of
the facilities for viewing; funeral service; merchandise selected such as
casket, outer burial container, memorial register, memorial folders and
acknowledgment cards, limousine or flower vehicle.
4. FORWARDING OF REMAINS TO ANOTHER FUNERAL HOME OR ANATOMICAL DONATION
This service includes: the basic professional services of
staff, the funeral director and use of basic facilities; filing and obtaining
legal documents and other paperwork necessary to complete the transfer;
transferring of the deceased from the place of death to the funeral home within
a 25 mile radius, embalming and/or sanitary care of the remains; other
preparation of the deceased. This charge also includes transportation to the
Anatomical Board.
This service does not include: visitation or viewing service;
merchandise selected such as casket, air tray or combination unit; memorial
register, memorial folders or acknowledgment cards; automotive equipment,
except as specifically set forth above; telephone and/or facsimile
communications; cash advances; air transportation charges; and/or other funeral
director charges.
5. RECEIVING OF REMAINS FROM ANOTHER FUNERAL HOME
This service includes: the basic professional services of the
funeral director and staff, administrative staff, and transportation to a local
cemetery.
This service does not include: transportation from airport,
embalming, visitation or funeral ceremony, merchandise selected such as casket,
outer burial container, memorial register, memorial folders and acknowledgment
cards or limousine.

CHAPEL IN THE MEADOWS MAUSOLEUMS
Forest Meadows takes pride in the continuing development and refinement that
ensures that our cemeteries maintains their status as Alachua County's most
beautiful and prestigious. We are proud of our beautiful "Chapel in the
Meadows" mausoleums complex.
Originally, mausoleum interment was only for royalty. In fact the word mausoleum
originated when the Persian queen Artimisia, in 452 B.C. created an elaborate
tomb to honor her husband, King Mausolus. The first mausoleum cost the
equivalent of 30 million dollars and required 10,000 men to build.
Today our "Chapel in the Meadows" mausoleum complex makes mausoleum interment a
possibility for every family, not just royalty. The first two phases contain
1,206 crypts, the majority of which have already been sold. The next
construction will feature phase three, which will contain 462 crypts. Surveys
have shown that 75% of people would prefer dry above ground mausoleum interment
if they could afford it. Well now they can!
While mausoleums in the past were only for royalty and the wealthy, Forest
Meadows has made dry above ground mausoleum interment available to
